Mental Health Assistant

We are seeking a skilled and compassionate Patient Care Coordinator to join our team at St Martins Hospital. As a key member of our team, you will be responsible for providing high-quality care and support to patients experiencing mental health difficulties.
About the RoleThe estimated salary for this position is £24,500 - £32,000 per annum, depending on experience. You will work alongside a dedicated team of nursing staff, providing physical health care and physical health checks as required or instructed, documenting the physical health checks on the early warning scoring system (NEWS2), charting, and reporting changes to the nurse/healthcare professional in charge.
Key Responsibilities:
- Assisting with daily tasks and activities as allocated by the Nurse/healthcare professional in charge.
- Providing information and support to patients, families, and carers.
- Contributing to the development and implementation of individualised care plans.

Probation Period and DBS Check
