About the Role:

We are seeking a qualified Social Worker to join our diverse and friendly NHS Continuing Healthcare Team in Hampshire. As a key member of our team, you will work collaboratively with Health colleagues to support individuals with their onward care journey following a Discharge to Assess period from hospital.

Undertaking assessments under the Care Act 2014 and for NHS CHC, you will work in partnership with individuals and their families to move them to appropriate care provision following assessment. Whether you are an experienced Social Worker looking to stretch yourself and take on a new challenge, or recently qualified looking to kickstart your career within NHS CHC, we can offer you a unique experience working alongside colleagues with a diverse range of skills, within a supportive space.

Key Responsibilities:

  • Undertake assessments under the Care Act 2014 and for NHS CHC
  • Work in partnership with individuals and their families to move them to appropriate care provision following assessment
  • Collaborate with Health colleagues to support individuals with their onward care journey
  • Develop and maintain relationships with individuals, families, and other stakeholders
  • Contribute to the development of the NHS Continuing Healthcare Team

Requirements:

  • Qualified Social Worker with UK-based Adults' Social Work experience
  • Relevant professional registration
  • Strong analytical and problem-solving skills
  • Ability to work collaboratively with colleagues and stakeholders
  • Commitment to learning and development
