Care Coordinator Position
SERVICE CARE SOLUTIONS LTD invites applications from skilled and empathetic care professionals to fill the role of Care Coordinator for Older Adults. This position involves managing a caseload of complex cases, performing assessments, and coordinating care plans with the multidisciplinary team.
About the Job
You will be working closely with patients who have varying mental health needs and comorbidities, ensuring that their care requirements are met through effective communication with healthcare providers and family members. The successful candidate will hold a valid PIN number and a fully enhanced DBS clearance.
Benefits Package
In addition to a competitive salary, you will receive weekly pay, free training opportunities, and a fully enhanced DBS check. Umbrella pay rates apply at £27 per hour, and a 'holiday pot' option allows you to accrue holiday time and reduce your hourly rate to £24.30. You can also earn a £250 bonus for starting a contract with SCS and a further £250 bonus per referred colleague.
