Pharmacist Role: Improving Community Health

As a Clinical Pharmacist, you will play a vital role in improving the health of the local community by utilizing your clinical skills to manage long-term conditions and support hospital discharge prescribing arrangements. You will work closely with GP practices to develop the clinical pharmacist role, providing specialist expertise in the use of medicines. Key responsibilities include taking responsibility for chronic disease management, undertaking structured medication reviews, and developing medicines management plans. You will also contribute to the Clinical Pharmacy Education Pathway and work closely with pharmacy professionals across Primary Care Networks.
Key Responsibilities:
* Manage chronic disease and undertake structured medication reviews to proactively manage patients with complex poly-pharmacy.
* Develop and manage medicines management plans and deliver patient services as determined by Network policy and local and national guidance.
* Maximize cost-effective prescribing and improve the quality of patient care.
* Provide specialist expertise in the use of medicines and help address public health and social care needs of patients.
* Develop relationships and work closely with other pharmacy professionals and the wider health and social care system.
* Take a central role in clinical aspects of shared care protocols, clinical research with medicines, and liaison with specialist pharmacists and community pharmacists.
