Rehabilitation Support Practitioner

At Central London Community Healthcare NHS Foundation Trust, we are seeking a highly motivated and compassionate Rehabilitation Support Practitioner to join our team. As a key member of our multidisciplinary team, you will play a vital role in supporting patients to achieve their rehabilitation goals.
**Key Responsibilities:**
- Work as part of a multi-disciplinary team to deliver high-quality rehabilitation services to patients with complex care needs
- Support patients to set and achieve their rehabilitation goals, promoting independence and self-management
- Deliver therapy programs, including exercise and education, to patients
- Communicate effectively with patients, carers, and healthcare professionals to ensure seamless care coordination
- NVQ 3 in Health or Social Care or equivalent academic qualification
- Previous experience in elderly care, preferably working with older people's rehabilitation in hospital or community settings
- Ability to work as a lone clinician and as part of a team
- Experience of working in an NHS environment
- Ability to deliver group and 1:1 exercise programs
