About the Role

We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our healthcare team, you will be responsible for delivering high-quality medical care to patients across a range of settings, including community hospitals and specialist services.

Main Responsibilities
  • Exercise advanced clinical expertise and decision-making skills in the assessment and management of patients with complex needs.
  • Work closely with medical, nursing, and therapy teams to ensure patients receive holistic care and safe discharge.
  • Lead on improvements to standards of care through supervision of practice, clinical audit, evidence-based practice, and facilitation of learning.
  • Develop and implement effective care plans, taking into account individual patient needs and preferences.
  • Collaborate with colleagues to ensure seamless care transitions and minimize avoidable delays.
Requirements
  • MSc in Advanced Clinical Practice, including Advanced Health Assessment skills at Level 7.
  • Non-medical prescribing qualification (V300) or ability to work within PGDs within scope of professional practice.
  • Advanced Assessment Skills Module.
  • Evidence of a portfolio of knowledge, skills, and professional development.
  • Ability to work flexibly and meet travel requirements of the role.
