**Clinical Specialist Practitioner - PCN/AAT Northgate Hospital**

**Would you like to work in a dynamic, creative and committed multi-disciplinary team? Are you a highly motivated, compassionate and dedicated mental health professional?**

We are excited to have an opportunity for a professional practitioner to join our newly established team in Great Yarmouth.

You must be able to demonstrate a warm, compassionate and needs-led approach to working with our service users.

The role includes multi-agency working alongside our Access and Assessment Team, providing support to our Primary Care Networks. Working alongside MHP's in GP surgeries and assessors within AAT supporting good clinical practice and continuing to build positive relationships across both services. You will have the opportunity to work with existing community mental health services, local outreach teams, social care and voluntary services to ensure service users' needs are best met.

As part of this team, responsibilities would include undertaking assessments, working collaboratively with individuals and others supporting agencies to manage needs, as well as holding a small caseload.

This role will give you the time and facilities to make a difference. You will have the ability to offer person-centered care while building trusting relationships to increase engagement with individuals.
- Completing assessments
- Supervision of band 6 staff
- Attending MDT's
- Attending interface meetings
- Supporting service manager with service improvements
- Complaints
- Quality improvement projects
