Job summary
We are looking for one enthusiastic, highly motivated Advanced Clinical Practitioner to join our patient focused and friendly practice team set within our purpose built surgery in the beautiful Malvern Hills.
We are a very supportive team committed to providing high quality patient care with a strong ethos on team working and integrated care.
Our experienced GPs will support and mentor you. We have an open, learning environment with regular staff meetings, audits and SEA meetings.
Main duties of the job
You will need to be a degree qualified Advanced Clinical Practitioner with relevant clinical skills and expertise to work autonomously to help provide ongoing clinical care and manage acute demand. You must have relevant experience and be an independent prescriber enabling you to independently assess, diagnose, treat and refer patients as appropriate. You will manage your own patient booked list on a daily basis, as well as taking on specific responsibilities in other areas of the practice.
About us
Our Surgery:
List size of 13,078 4 GP partner practice with 5 salaried GPs and 2 GP trainees. They are supported by management team, reception team, clinical pharmacist, paramedic, social prescribers and a practice nursing team. EMIS webWe have recently moved over to a total triage model:
Total triage means that every patient contacting the practice is first triaged before making an appointment by a GP Partner. The purpose of total triage is to get patient needs met by the most appropriate person or service with the minimum amount of handoff. The total triage model aims to help practices meet patient demand within their existing capacity by using the skill-mix of the wider team, utilising online services, telephone and video consultations as well as face-to-face consultations when needed. It explicitly acknowledges that a large proportion of the need that presents to general practice does not need a general practitioner to deal with it, and in fact is likely to be better managed by another person, service or by the patient themselves if they have access to the right information. The three critical steps in a triage model of general practice are: asking patients what they need, signposting by administrative teams and clinical triage. Job description
Job responsibilities
You will need to manage your daily appointments list fully, prescribing and making any necessary onward referrals or actions as required.
Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary and secondary health care team. Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations. Advise patients on general health care and their continuing medical needs, with referral to other members of the primary and secondary health care team as necessary. Participate in the administrative and professional responsibilities of the practice team. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes at the time of the consultation. Attend and participate in practice meetings as required. Be able to work flexible hours when necessary, to accommodate holidays, sickness and vaccine clinics.
