About the Role

We are seeking a dedicated and compassionate Care Team Leader to join our team at Sway Place in the beautiful New Forest National Park. As a Care Team Leader, you will be responsible for leading and supervising your shift, case managing residents, and liaising with families and healthcare professionals to ensure the best care plans are created and delivered.

Key Responsibilities

  • Leading and supervising your shift to ensure the highest standards of care are maintained
  • Case managing residents and their families to ensure their needs are met and their care plans are delivered
  • Administering and recording medication, and carrying out risk assessments to ensure the safety and well-being of residents
  • Working closely with the leadership team to develop services and improve the quality of care
  • Providing a fun, inclusive, and professional approach at all times, and promoting a positive and supportive team culture

About You

  • NVQ Level 3 in health and social care desirable
  • Proven ability to lead and supervise others, and a good understanding of care planning processes
  • Experience in administering medication, and a willingness to undertake training
  • A natural ability to help make the lives of others more fulfilling, and a passion for delivering high-quality care
