Job Title: Care Manager Liaison

We are seeking a dedicated Care Manager Liaison to join our team at Service Care Solutions. This role involves supporting Adults and Carers within the local community by undertaking assessments, providing practical support, and information, developing support plans, and ensuring effective delivery of services.

Main Responsibilities:
• Conduct assessments of Adults and Carers in line with relevant legislation, policy, and procedures.
• Provide advice, support, and information to Adults and Carers.
• Collaborate with individuals to develop support plans and arrange/monitor service delivery and payments.
• Work closely with colleagues and other professional staff to ensure service users' needs are met.
• Monitor and review services for timely delivery in line with assessed needs.

Requirements:
• Previous experience working in Health and Social Care, particularly with Adults and Carers.
• Demonstrated knowledge and understanding of the Care Act 2014, willingness to learn more.
• Understanding of or willingness to learn about the Mental Capacity Act 2005 and other relevant legislation, policies, or procedures concerning Adults.

Working Hours: 36 hours per week. The estimated salary is £23 umbrella an hour (£18 PAYE an hour).
