Rehabilitation Case Manager

We are seeking a highly skilled Rehabilitation Case Manager to join our team at Health and Care Management Ltd. As a key member of our clinical team, you will be responsible for delivering high-quality rehabilitation services to our clients.
The ideal candidate will have a strong background in sports therapy or sports rehabilitation, with a minimum of 1 year's clinical experience. You will be required to possess excellent communication skills, with the ability to work effectively with patients, colleagues, and other stakeholders.
Key Responsibilities:
- Deliver clinically robust telephone assessments to inform treatment plans
- Assess, plan, implement, and evaluate interventions to achieve optimal patient outcomes
- Utilize the bio-psychosocial model to inform clinical decisions and patient interactions
- Provide education and clinical support to patients throughout their rehabilitation journey
Essential Skills:
- Relevant clinical degree (sport therapy, sport rehabilitation, or physiotherapy)
- 1 year's clinical experience or previous case management expertise
- Current governing registrations (e.g., BASRaT or SST, HCPC)
- Excellent written and verbal communication skills
