Rehabilitation Case Manager

We are seeking a highly skilled Rehabilitation Case Manager to join our team at Health & Case Management Rehabilitation Ltd. As a key member of our team, you will play a vital role in delivering high-quality rehabilitation services to our clients.
Key Responsibilities:- Conduct comprehensive telephone assessments to identify client needs and develop effective rehabilitation plans.
- Collaborate with clients, healthcare professionals, and other stakeholders to ensure seamless care coordination and optimal outcomes.
- Utilize the bio-psychosocial model to inform clinical decisions and provide patient-centered care.
- Develop and maintain strong relationships with clients, families, and healthcare providers to ensure consistent and high-quality care.
- Participate in regular case reviews and quality improvement initiatives to ensure best practices and continuous improvement.
- Relevant clinical degree (sport therapy, sport rehabilitation, or physiotherapy)
- Current governing registrations of your relevant professional body (e.g., BASRaT or SST, HCPC)
- Excellent written and verbal communication skills, good attention to detail, and time management skills
