We are seeking a skilled Rehabilitation Case Manager to join our team at Health Case Management Limited (HCML). As a Rehabilitation Case Manager, you will be responsible for delivering clinically robust, consistent, and comprehensive telephone assessments that lead to the most clinically appropriate outcomes. This role requires a high level of communication to influence and support a patient's journey, while maintaining great attention to detail within a fast-paced environment.

Key Responsibilities:

* Deliver clinically robust, consistent, and comprehensive telephone assessments
* Assess, plan, implement, and evaluate appropriate interventions and clinical pathways
* Utilize the bio-psychosocial model when making contact with patients and with clinical decisions
* Educate and provide clinical support to patients

Essential Skills:

* Relevant clinical degree (sport therapy, sport rehabilitation, or physiotherapy)
* 1 year of clinical/hands-on experience or previous case management or vocational rehabilitation expertise
* Current governing registrations of your relevant professional body
* Excellent written and verbal communication skills, good attention to detail, and time management skills
