About the Role

We are seeking a highly skilled Rehabilitation Case Manager to join our team at Health and Care Management Ltd. As a key member of our team, you will play a vital role in delivering high-quality rehabilitation services to our clients.

The successful candidate will have a background in sports therapy or sports rehabilitation and will be responsible for providing clinically robust, consistent, and comprehensive telephone assessments to ensure the most effective outcomes for our clients.

You will be responsible for assessing, planning, implementing, and evaluating appropriate interventions and clinical pathways to expedite patient recovery and case closure. Your excellent communication skills and attention to detail will enable you to work effectively in a fast-paced environment, often with competing priorities.

This is an excellent opportunity for a motivated and experienced professional to utilize their hands-on experience in a telephonic, office-based environment and make a real difference in the lives of our clients.

Key Responsibilities
  • Deliver clinically robust, consistent, and comprehensive telephone assessments
  • Assess, plan, implement, and evaluate appropriate interventions and clinical pathways
  • Regularly use the bio-psychosocial model when making contact with patients and with clinical decisions
  • Utilize every contact point with patients as a chance to educate and provide clinical support
  • Shared responsibility for patient cases, supporting colleagues proactively and encouraging a team working environment
  • Assist with optimizing service delivery
  • Achieve the highest standards of patient care, including the consistent provision of clinical guidance and accurate, clear T&Cs advice
Requirements
  • Relevant clinical degree (sport therapy, sport rehabilitation, or physiotherapy)
  • 1 year's clinical/hands-on experience or previous case management or vocational rehabilitation expertise
  • Current governing registrations of your relevant professional body (e.g., BASRaT or SST, HCPC body)
  • Excellent written and verbal communication skills, good attention to detail, and time management skills
