About the Role

We are seeking a highly skilled Rehabilitation Case Manager to join our team. As a key member of our clinical services team, you will play a vital role in delivering high-quality rehabilitation services to our clients.

Key Responsibilities
  • Deliver comprehensive telephone assessments to identify client needs and develop effective rehabilitation plans.
  • Work closely with clients, healthcare professionals, and other stakeholders to ensure seamless care coordination and optimal outcomes.
  • Utilize the bio-psychosocial model to inform clinical decisions and promote patient-centered care.
  • Provide education and support to clients to empower them to manage their recovery and achieve their goals.
Essential Skills
  • Relevant clinical degree (e.g., sport therapy, physiotherapy) and 1 year of clinical experience or equivalent case management expertise.
  • Current governing registrations (e.g., BASRaT, HCPC) or ability to obtain membership quickly.
  • Excellent written and verbal communication skills, with a strong attention to detail and time management.
