About the Role

We are seeking a highly motivated and enthusiastic mental health nurse to join our Community Mental Health Team (CMHT) in Maidstone. As a key member of the multidisciplinary care team, you will have continued responsibility to assess, plan, coordinate, deliver, and evaluate care, communicate findings, influence, and drive change, and promote health and best practice, patient safety, and experience.

Main Duties
  • Assess, plan, coordinate, deliver, and evaluate care for service users
  • Communicate findings, influence, and drive change to promote health and best practice, patient safety, and experience
  • Work collaboratively with the multidisciplinary care team to provide comprehensive multi-disciplinary services
  • Develop and maintain effective relationships with service users, carers, and other stakeholders
  • Contribute to the development and implementation of clinical care pathways

Person Specification
  • Registered on the NMC
  • One year experience at Band 5 or above in hospital or community healthcare
  • Evidence of continued professional development, appropriate to role
  • Clear understanding of CPA and care coordination role
