Job Summary:
We are seeking a highly motivated and experienced Community Mental Health Nurse to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to individuals with mental health needs in the community.
Key Responsibilities:
- Assess and plan care for individuals with mental health needs, working collaboratively with the multidisciplinary care team.
- Liaise with Clinical Pathway Leads to ensure seamless care coordination.
- Deliver group-based interventions and promote health and best practice.
- Influence and drive change to improve patient outcomes.
Requirements:
- Registered on the NMC register.
- Two years' experience at Band 5 or above in hospital or community healthcare.
- Evidence of continued professional development, appropriate to the role.
