About the Role

We are seeking a highly motivated and compassionate Health and Social Care Support Worker to join our team at Livewell Southwest. As a key member of our team, you will play a vital role in providing holistic care to patients with the aim of reablement.

Key Responsibilities
  • Provide support to patients with activities of daily living, promoting independence and rehabilitation in a safe manner.
  • Work collaboratively with the multidisciplinary team to deliver care as prescribed by registered professionals.
  • Support the gathering of information from various sources, including hospital discharge assessments and discharge summaries.
  • Assist with medication administration according to patient needs, in line with STCC processes.
  • Engage with patients to promote engagement in everyday life skills, promoting independence and rehabilitation.
  • Follow Livewell Southwest policies and clinical guidance related to STCC.
  • Take reasonable care for personal health and safety, and be aware of Health and Safety at Work Act implications.
Person Specification
  • Vocational Qualification at level III or equivalent, with ability to demonstrate skills, knowledge, and ability to work at that level.
  • GCSE Maths and English at grade C/4 or above, or hold level 2 Functional skills qualifications in English and Maths.
  • NVQ 2 or 3 or equivalent level of competence.
  • Continual assessment of patient needs, with knowledge of Livewell Southwest policies and Mental Health Act awareness.
  • Ability to engage patients in activities, with understanding of role and evidence of literacy and numeracy skills.
  • Demonstrates an understanding of patients' physical, emotional, social, and spiritual needs according to speciality.
  • Awareness of personal safety, care plans, and risk assessments, with confidentiality and record-keeping skills.
