About the Role
We are seeking a highly motivated and compassionate Care Assistant to join our team at Livewell Southwest. As a Care Assistant, you will play a vital role in providing holistic care to patients with the aim of reablement, promoting independence and rehabilitation.
Key Responsibilities
- Provide holistic care to patients, including support with activities of daily living, medication administration, and clinical observations.
- Work collaboratively with the multidisciplinary team to deliver care plans and risk assessments.
- Support patients to engage in activities that promote independence and rehabilitation.
- Maintain accurate and confidential records of patient care.
- Participate in training and development opportunities to maintain clinical competencies.
- Contribute to the audit process and quality improvement initiatives.
Requirements
- Vocational Qualification at level III or equivalent.
- GCSE Maths and English at grade C/4 or above, or level 2 Functional skills qualifications in English and Maths.
- NVQ 2 or 3 or equivalent level of competence.
- Experience of working in an Adult Social or Health setting, preferably in a multidisciplinary team.
- Ability to use own initiative and work unsupervised.
