About the Role

We are seeking a highly motivated and compassionate Care Assistant to join our team at Livewell Southwest. As a Care Assistant, you will play a vital role in providing holistic care to patients with the aim of reablement, promoting independence and rehabilitation.

Key Responsibilities

  • Provide holistic care to patients, including support with activities of daily living, medication administration, and clinical observations.
  • Work collaboratively with the multidisciplinary team to deliver care plans and risk assessments.
  • Support patients to engage in activities that promote independence and rehabilitation.
  • Maintain accurate records and reports, including patient care plans and incident reports.
  • Participate in training and development opportunities to maintain clinical competencies and enhance skills.
  • Contribute to the development of the team and the organization, including participating in audits and quality improvement initiatives.

Requirements

  • Vocational Qualification at level III or equivalent, with experience of working in an adult social or health setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
  • Ability to work independently and as part of a team, with a high level of flexibility and adaptability.
  • Knowledge of Livewell Southwest policies and procedures, including confidentiality, data protection, and health and safety.
  • Ability to maintain accurate records and reports, with a high level of attention to detail.
