The Discharge Ward Coordinator plays a crucial role in supporting the nursing teams at York and Scarborough Teaching Hospitals NHS Foundation Trust. As a key member of our team, you will be responsible for coordinating the discharge process, ensuring that patients receive the necessary support to safely transition back home. This includes assessing patient care needs and developing, implementing, and evaluating programs of care.

Main Responsibilities
  • To work closely with the Ward/Department Sister and the team to ensure high standards of nursing care are provided at all times
  • To maintain hospital cleanliness standards on their ward/department

This is a great opportunity for the right candidate to develop their leadership skills while working as part of an innovative nursing team. The successful candidate will undergo Leadership training and development.

Salary and Benefits
  • £35,000 - £40,000 per annum, depending on experience
  • Access to the NHS Pension Scheme with generous benefits upon retirement, including a lump sum and pension for dependents
  • 27 days holiday rising to 33 days (depending on NHS Trust service)
