Job Title: Community Care Support Specialist

About the Role:

We are seeking a dedicated and compassionate Community Care Support Specialist to join our team at P2P Specialist Nurses. As a Community Care Support Specialist, you will be responsible for providing essential care and support to individuals in their own homes and communities.

Key Responsibilities:

* Providing companionship and emotional support to promote well-being and reduce isolation.

* Assisting clients with daily tasks such as personal care, meal preparation, medication prompts, and mobility support.

* Supporting clients in maintaining their independence within their homes and communities.

* Following care plans and ensuring the safety and dignity of each client is prioritised.

* Travelling between client homes in your own vehicle, ensuring timely care is provided.

Requirements:

* A valid UK driving licence and access to your own vehicle.

* Previous experience in a care or support role is desirable but not essential (full training provided).

* Willingness to travel between client locations.
