Role Summary:
As a Community Care Support Worker, you will be responsible for providing essential care and support to individuals in their own homes, helping them to maintain independence and dignity. You will travel between clients, ensuring they receive the best possible care in line with their personalised care plans.

Key Responsibilities:

* Assisting clients with daily tasks such as personal care, meal preparation, medication prompts, and mobility support.

* Providing companionship and emotional support to promote well-being and reduce isolation.

* Supporting clients in maintaining their independence within their homes and communities.

* Following care plans and ensuring the safety and dignity of each client is prioritised.

* Travelling between client homes in your own vehicle, ensuring timely care is provided.

* Reporting any changes in clients’ conditions to the care team and updating records as necessary.

Requirements:

* Essential: A valid UK driving licence and access to your own vehicle.

* Previous experience in a care or support role is desirable but not essential (full training provided).

* A caring, empathetic, and patient nature.

* Strong communication skills and the ability to build rapport with clients.

* Willingness to travel between client locations.

* Ability to work independently and manage your time effectively.
