We are seeking a highly skilled and motivated Diabetes Specialist Nurse to join our team at BHRUT NHS. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to adults with diabetes, working closely with patients, relatives, and healthcare professionals to achieve optimal outcomes.
Main Duties
- Assess and manage patients with diabetes, providing individualized care plans and education to promote self-management and improve health outcomes.
- Collaborate with the multidisciplinary team to develop and implement effective diabetes care pathways, ensuring seamless transitions between hospital and community settings.
- Provide education and support to patients, relatives, and healthcare professionals, promoting best practice and evidence-based care.
- Contribute to the development and implementation of quality improvement initiatives, ensuring continuous improvement in diabetes care.
- Work closely with the diabetes team to ensure effective communication and coordination of care, promoting a culture of collaboration and teamwork.

Person Specification
- Registered General Nurse (RGN) with a relevant qualification in diabetes care.
- Experience in diabetes care, with a strong understanding of diabetes pathophysiology, treatment options, and management strategies.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, relatives, and healthcare professionals.
- Ability to work autonomously and as part of a team, with a strong commitment to continuous learning and professional development.
