Job Overview

We are seeking a highly skilled and motivated Diabetes Specialist Nurse to join our team at Barking Havering and Redbridge University Hospital Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to adults with diabetes, working closely with patients, relatives, and healthcare professionals to achieve optimal outcomes.

Main Duties

  • Assess and manage patients with diabetes, providing education and support to promote self-management and improve health outcomes.
  • Develop and implement individualized care plans, working collaboratively with the multidisciplinary team to ensure seamless care.
  • Conduct audits and quality improvement initiatives to enhance diabetes care and services.
  • Provide education and training to staff, patients, and carers on diabetes management and prevention.
  • Collaborate with other healthcare professionals to develop and implement protocols and guidelines for diabetes care.

Person Specification

Education / Qualifications

  • RGN level 1
  • Relevant qualification applicable to diabetes
  • Education to degree level or equivalent experience

Knowledge / Experience

  • Experience in audit

You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020, and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
