About the Role
We are seeking an experienced Mental Health Coach to join our team at Mayflower Medical Group, part of Fuller and Forbes Partnership. As a valued member of our multidisciplinary team, you will play a vital role in shaping the delivery of contemporary health support and treatment across our practices.
Job Summary
The successful candidate will be responsible for providing individualised support to patients, exploring ways to improve mental health and wellbeing. You will work closely with local teams to address the wider determinants of health and deliver tailored packages of support and education.
Main Responsibilities
- To provide one-to-one support for individuals, helping them explore ways to improve their mental health and wellbeing
- To utilise self-help and facilitated tools to aid individual goal planning and recovery measurement
- To potentially support individuals in making informed choices about their health and wellbeing, including improving physical activity levels, promoting healthy diets, and enhancing mental wellbeing
- To support access to wider community activities and services as part of individual plans
- To effectively signpost and support access to appropriate services
- To support primary care colleagues in addressing wider health determinants and delivering individual packages of support and education
- To facilitate collective action, support, and advice to people within local communities to reduce health inequalities while responding to identified health needs through one-to-one support
- To utilise social prescribing to develop individual recovery and support planning, working directly with the team across the practices
- To promote effective and efficient working practices across the team to achieve measurable outcomes
- To plan own workload and promote innovation and continuous improvement to deliver better services for service users and patients
Requirements
To be successful in this role, you will need:
- Experience within a healthcare setting or similar role
- Experience in advising others in a caring background
- Demonstrable commitment to professional development
- Able to work effectively in a multidisciplinary team environment
- Familiarity with SystmOne Web clinical system (desirable)
Working Environment
Mayflower Medical Group is a collection of 5 GP Surgeries based across Plymouth, serving over 35,000 patients. Our vision is to enable patients to live longer and healthier lives that are full, active, and meaningful. We aim to do this by developing a sustainable primary healthcare service, working closely with our multidisciplinary team and PCN staff.
Salary & Benefits
We offer a competitive salary range of £25,500 - £31,300 per annum, depending on experience, plus excellent benefits package, including opportunities for professional development and growth within our organisation.
