About the Role

We are seeking a highly skilled and experienced Practice Nurse to join our team at a large, busy, modern GP Practice in Wales. The successful candidate will work with a list size of 25,000 patients and join a team of four Practice Nurses, four Health Care Support Workers, and two phlebotomists.

Main Responsibilities

The post-holder will provide clinical support in areas such as diabetes, respiratory, women's health, and family planning, as well as wound management and immunisations. Key duties will include:

  • Chronic disease management, including diabetes, respiratory, and women's health
  • Wound management and care
  • Immunisation and vaccination programmes
  • Women's health and family planning services
  • Adherence to Infection Control procedures
  • Taking and recording observations, such as blood pressure monitoring
  • Venepuncture and phlebotomy

About Us

We are one of the largest GP practices in Wales, serving over 25,000 patients across three sites. Our team includes seven partners, eight salaried GPs, five nurses, four HCAs, and a clinical pharmacist. We offer a range of services, including first contact physio and GP Support officers who deal with housing, benefits, social services queries, and social prescribing.

Person Specification

The ideal candidate will have:

  • Significant post-qualification experience as a Registered Nurse
  • Excellent communication skills and ability to work with staff at all levels
  • Experience in chronic disease management, immunisation, and venepuncture
  • Ability to perform ECGs and other clinical procedures
