Care Coordinator in Lincolnshire

Job Description:
We are recruiting for a Care Coordinator to join our team in Lincolnshire. The successful candidate will have experience of working with adults, conducting assessments and developing support plans to promote independence.
Main Responsibilities:
- Conduct complex assessments to determine eligibility for support services.
- Develop and implement support plans to promote independence and well-being.
- Provide listenable support to individuals in crisis, working collaboratively to achieve effective plans.
- Manage section 42 enquiries and complex situations, utilising professional judgement and expertise.
- Collaborate with teams and services to provide a seamless service, promoting knowledge of local resources.
- Participate actively in the team, supporting colleagues and contributing to service development.
- Deliver against team and individual performance targets, taking ownership of responsibilities.
- Contribute to service improvement through communication, feedback, and collaboration.
This role offers a competitive salary of £38,000 - £45,000 per annum, depending on experience. You will also benefit from a comprehensive benefits package, including health insurance and pension scheme, as well as ongoing training and development opportunities.
