Job Description:

We are recruiting for a Care Coordinator to join our team in Lincolnshire. The successful candidate will have experience of working with adults, conducting assessments and developing support plans to promote independence.

Main Responsibilities:

  • Conduct complex assessments to determine eligibility for support services.
  • Develop and implement support plans to promote independence and well-being.
  • Provide listenable support to individuals in crisis, working collaboratively to achieve effective plans.
  • Manage section 42 enquiries and complex situations, utilising professional judgement and expertise.
  • Collaborate with teams and services to provide a seamless service, promoting knowledge of local resources.
  • Participate actively in the team, supporting colleagues and contributing to service development.
  • Deliver against team and individual performance targets, taking ownership of responsibilities.
  • Contribute to service improvement through communication, feedback, and collaboration.

This role offers a competitive salary of £38,000 - £45,000 per annum, depending on experience. You will also benefit from a comprehensive benefits package, including health insurance and pension scheme, as well as ongoing training and development opportunities.
