About the Position

We are looking for a motivated and empathetic Care Coordinator to join our team at Dimensions. This is a full-time position, with an estimated salary of £12 per hour (£50 per Sleep-In Shift) and a comprehensive benefits package, including generous leave and opportunities for career development.

As a Care Coordinator, you will work closely with our multidisciplinary team to develop personalized care plans and ensure that each individual receives the highest level of care and support. You will also have the opportunity to engage in creative pursuits, such as gardening, arts and crafts, and sensory sessions, which cater to the interests and preferences of our service users.

About the Role

Your responsibilities will include empowering independence, engaging in activities, providing care, and adapting to the varying needs of those you support. You will also participate in morning, afternoon, and evening shifts, including sleep-ins, to ensure that our service users receive 24-hour support.

You will work closely with our team to ensure that each individual's needs are met and that they receive the highest level of care and support. This includes developing personalized care plans, participating in creative pursuits, and providing 1:1 support to ensure that each individual feels safe, respected, and encouraged in all aspects of their lives.

  • Empowering Independence: Assist with personal care, stoma care, medication administration (including epilepsy buccal), and daily living activities while respecting each individual's spatial awareness and personal preferences.
  • Engaging in Activities: Plan and participate in outings and activities that cater to their interests, such as drives, arts and crafts, and sensory sessions.
  • Providing Care: Offer 1:1 support, ensuring they feel safe, respected, and encouraged in all aspects of their lives.
  • Flexible & Forward-Thinking: Adapt to the varying needs of those you support, thinking creatively to enhance their day-to-day experiences.
  • Building Strong Relationships: Develop a trusting bond with the individuals and maintain close contact with their families, ensuring open communication and a supportive environment.
