We are seeking a Care Coordinator to join our team.
- 12 months scheduling experience - essential
- Sound knowledge of the area
- Must be a driver, with own car
- Level 2 in Health & Social Care - Preferred
- Forward-thinker and problem solver
- Care delivery experience - essential
- Open with your feeling and thoughts
- Knowledge of Microsoft Office
- Able to adapt well to change

**Your Duties**
- Schedule Visits
- Process Annual Leave Requests
- Conduct back to work interviews
- Liaise with Clients, their family, Care Pro’s and other agencies
- Delivery moving and handling training
- Collate payroll data
- Match Care Pro's to clients dependant on needs
- Schedule & conduct introductions, shadowing & team meetings
- Participate with On-Call duties
- Field new client enquires
- Deliver care when required (this could be outside of office hours)
- Be proactive to scheduling changes

What you need to know:

- Working Pattern: Monday to Friday 08:30 till 17:00
- On-Call one evening per week, occasional paid weekend due to business needs
- Salary £22,500
- Annual Leave: 21 Days + Bank Holidays

**We don’t offer sponsorship, however other offices in the Home Instead Network might.**
