We are a leading provider of complex care packages within the community, specialising in adult complex care. Our company is based in South West London and has secured further packages during an exciting growth phase.

Job Summary:

This is a challenging and rewarding role for a skilled Adult Nurse Manager who can provide face-to-face support to adult clients and their families, whilst creating personalised care plans for each person.

Responsibilities:
  1. Design and deliver tailored care plans specific to individual client needs
  2. Effectively manage multidisciplinary teams through strong communication and teamwork
  3. Provide ongoing monitoring, support, and assessment of client physical and physiological needs
  4. Deliver nursing interventions including management of medicine delivery
  5. Build and maintain sophisticated relationships with client families
  6. Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
Requirements:
  1. RGN with active NMC PIN
  2. Supportive management style that brings out best in others
  3. Up-to-date experience of tracheostomy care and ventilators within last 2 years
  4. Advanced communication skills verbally and written to liaise effectively with patients, families, and MDT
  5. Suitable for Band 5/Band 6 Nurses with evidence of effective leadership and management of HCAs
  6. Full UK driving licence desirable but not essential

  1. Estimated salary: up to £45,000 per annum DOE
