Complex Care Lead

- Design and deliver a tailored care plan specific to an individual client's needs.
- Effectively manage the multidisciplinary team (MDT) through strong communication and teamwork.
- Provide ongoing monitoring, support, and assessment of client's physical and physiological needs within your caseload.
- Deliver nursing interventions including the management of medicine delivery.
In return for your expertise and dedication, you will receive a competitive salary of up to £55,000 per annum DOE, 1 day working from home per week/4 days in the community, Monday to Friday (9-5) hours, 33 days holiday, private pension scheme, use of a company car or travel card for community travel, business travel expenses of £0.45 per mile, and an excellent Learning and Development programme.
