About the Job

Job Description

As a Primary Care Nurse, your responsibilities will vary. You will work within a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions. You will use your skills and knowledge to deliver nurse-led clinics to your patient group while being supported by a multi-disciplinary team.

We are looking for caring, compassionate, but also driven professionals who can help us drive our vision for fair and inclusive healthcare access for all. To succeed in this role, you will need:

  • Registered nurse - Adult
  • Current professional qualification (NMC)
  • Evidence of Continued Professional Development
  • Understanding of the complexities of multi-disciplinary and multi-agency working
  • Understanding of clinical governance, health and safety, QOF, EBP, and national standards
