Primary Care Nurse Professional

We are seeking a highly skilled and dedicated Primary Care Nurse to join our team at HMP Stafford in Stafford. As a Primary Care Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities:
- To work within a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions.
- To use your skills and knowledge to deliver nurse-led clinics to your patient group while being supported by a multi-disciplinary team.
Requirements:
- A Registered Adult Nurse with current professional qualification (NMC) and evidence of Continued Professional Development.
- An understanding of clinical governance, health and safety, QOF, EBP, and national standards.
