Job Description: As a Mental Health Support Worker, you will play a vital role in supporting individuals to live independently, manage their mental health, and achieve their personal goals. You will work closely with a team of healthcare professionals to provide practical and emotional support, promote recovery, and assist with day-to-day activities tailored to each person's needs.
Main Responsibilities:
- Empower individuals to manage their mental health through person-centered support
- Assist with daily activities such as meal preparation, medication management, budgeting, and attending appointments
- Promote social inclusion by facilitating community involvement and social opportunities to reduce isolation and build meaningful connections
- Collaborate with healthcare professionals to create and implement care plans
- Maintain accurate records, write progress notes, and report concerns or changes in an individual's mental health condition
