Community Support Worker (Mental Health)

About the Role: We are seeking an experienced and compassionate Community Support Worker to join our mental health team. The successful candidate will provide support and guidance to individuals with mental health conditions, enabling them to live independently and achieve their goals.

Main Responsibilities:
  • Conduct home visits and assessments to identify individual needs and develop personalized support plans.
  • Provide practical support with daily tasks, such as household chores, meal preparation, and personal care.
  • Facilitate access to community resources, services, and activities that promote social inclusion and well-being.
  • Collaborate with healthcare professionals to coordinate care and ensure effective service delivery.
  • Maintain accurate records and report any concerns or incidents.

Requirements:
  • Diploma in Health and Social Care or equivalent qualification.
  • Proven experience working with vulnerable adults, particularly those with mental health conditions.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively within a team environment.
  • Flexibility to work varied shifts, including evenings and weekends.
