Responsibilities and Requirements:

  • Develop and implement effective support plans to promote positive change
  • Work collaboratively with other professionals to ensure seamless service delivery
  • DipSW or equivalent qualification
  • Registered with HCPC
  • Current DBS clearance
  • Valid driving license
