Supportive Community Care Coordinator

We are seeking a skilled and compassionate Supportive Community Care Coordinator to join our team. The successful candidate will be responsible for coordinating the delivery of high-quality, person-centred care to individuals with mental health needs, working closely with the multidisciplinary team to develop and implement individualised care plans.
About the Role:
- Coordinate the delivery of a range of therapeutic interventions and support services to individuals with mental health needs
- Work collaboratively with the multidisciplinary team to develop and implement individualised care plans
- Provide emotional support and guidance to individuals and their families
- Contribute to the development and implementation of new initiatives and projects to improve mental health services
Salary: £28,000 - £34,000 per annum (dependent on experience)
