Responsibilities and Expectations:
* Provide daily living assistance, including meal preparation, household chores, and personal care.* Develop and implement personalized care plans to meet the unique needs of each service user.* Monitor progress and adjust care plans as needed to ensure optimal outcomes.* Facilitate community and social outings to promote social interaction and engagement.* Collaborate with colleagues to ensure seamless delivery of support services.
