Job Summary

We are seeking a highly organized and detail-oriented Clinical Pathway Coordinator to join our Frailty Service team at East Kent Hospitals University NHS Foundation Trust. As a key member of our team, you will play a vital role in ensuring the smooth running of our frailty service, providing administrative support to our multidisciplinary team, and contributing to the delivery of high-quality patient care.

Main Responsibilities
  • Provide administrative support to the frailty multidisciplinary team, including coordination of clinical and non-clinical activities
  • Manage and maintain effective systems, including paper-based and electronic records
  • Prepare and send correspondence, including SBAR referrals to community services
  • Attend meetings, take minutes, and arrange follow-up actions
  • Book appointments and manage investigation lists for frail and HCOOP patients
  • Collate and present activity data required for the frailty service
