About the Role
We are seeking a highly organized and detail-oriented Clinical Pathway Coordinator to join our Frailty Service team at East Kent Hospitals University NHS Foundation Trust. As a key member of our team, you will play a vital role in ensuring the smooth running of our frailty service, providing administrative support to our multidisciplinary team, and coordinating clinical and non-clinical activities.
Main Responsibilities
- Provide administrative support to the frailty MDT, including coordination of clinical and non-clinical activities
- Manage and maintain effective systems, including paper-based and electronic documentation
- Coordinate and arrange meetings, appointments, and follow-ups with patients and community services
- Prepare and send correspondence, including SBAR referrals to community services
- Collect and present activity data required for the frailty service
