Clinical Pathway Coordinator

About the Role
The Frailty Clinical Pathway Coordinator will play a crucial role in ensuring the smooth running of the frailty service, both on the Frailty Assessment Unit and the wider frailty caseload. This includes providing full and comprehensive administrative support to the frailty Multi-Disciplinary Team (MDT), including coordination of clinical and non-clinical activities on the unit and within the Emergency Department and wards.
Main Responsibilities
- Undertake clerical tasks supporting the frailty MDT, including telephone enquiries and the collection of patient details and history.
- Upload clinical documentation and send SBAR referrals to community services.
- Attend Teams meetings with community services and arrange and attend MDT meetings, minute these.
- Book Frailty Hot Clinic appointments and hold investigation lists for Frail and HCOOP patients.
- Arrange follow-up with patients and collect, collate, and present activity data required.
- Book teaching rooms and meeting rooms, and prepare papers.
- Ensure effective time management and forward planning of the frailty service.
