About the Role
We are seeking a highly organized and detail-oriented Clinical Pathway Coordinator to join our Frailty Service team at East Kent Hospitals University NHS Foundation Trust. As a key member of our team, you will play a vital role in ensuring the smooth running of our frailty service, providing administrative support to our multidisciplinary team, and contributing to the delivery of high-quality patient care.
Main Responsibilities
- Provide administrative support to the frailty multidisciplinary team, including coordination of clinical and non-clinical activities
- Manage and maintain effective systems, including paper-based and electronic records
- Prepare and send correspondence, including SBAR referrals to community services
- Attend meetings, take minutes, and maintain accurate records
- Coordinate the booking of Frailty Hot Clinic appointments and manage investigation lists for frail and HCOOP patients
- Collate and present activity data required for the frailty service
