About the Role

The Frailty Clinical Pathway Coordinator will play a vital role in ensuring the smooth operation of the Frailty Service at East Kent Hospitals University NHS Foundation Trust. This includes providing administrative support to the Frailty Multi-Disciplinary Team (MDT) and coordinating clinical and non-clinical activities on the unit and within the Emergency Department and wards.

Main Responsibilities

  • Provide clerical support to the Frailty MDT, including telephone enquiries, data collection, and documentation uploading.
  • Coordinate referrals to community services and attend meetings with community teams.
  • Arrange and minute MDT meetings, book appointments, and manage investigation lists for Frail and HCOOP patients.
  • Collect and present activity data, and ensure effective time management and forward planning for the Frailty Service.
