Job Summary

We are seeking a highly organized and detail-oriented Clinical Pathway Coordinator to join our Frailty Service team at East Kent Hospitals University NHS Foundation Trust. As a Clinical Pathway Coordinator, you will play a crucial role in ensuring the smooth running of our Frailty Service, providing administrative support to the Frailty Multidisciplinary Team (MDT), and coordinating clinical and non-clinical activities.

Main Duties
  • Provide administrative support to the Frailty MDT, including clerical tasks, telephone enquiries, and data collection.
  • Coordinate the uploading of clinical documentation, sending of SBAR referrals to community services, and attending Teams meetings with Community services.
  • Arrange and attend MDT meetings, minute these, and book Frailty Hot Clinic appointments.
  • Hold investigation lists for Frail and HCOOP patients, arrange follow-up with patients, and collect, collate, and present activity data required.
  • Book teaching rooms and meeting rooms, prepare papers, and ensure effective time management and forward planning of the Frailty service.
