We are seeking a highly organized and detail-oriented Clinical Pathway Coordinator to join our Frailty Service team at East Kent Hospitals University NHS Foundation Trust. As a key member of our team, you will play a vital role in ensuring the smooth running of our frailty service, providing administrative support to our multidisciplinary team, and coordinating clinical and non-clinical activities.

Main Responsibilities

  • Provide administrative support to the frailty multidisciplinary team, including coordination of clinical and non-clinical activities
  • Manage and maintain effective systems, including paper-based and electronic documentation
  • Coordinate and arrange meetings, appointments, and follow-up activities with patients and community services
  • Collect and collate patient data and activity information for reporting purposes
  • Support the development and implementation of effective systems and processes to improve patient care and outcomes
