**Job Description:**

We are seeking a Palliative Care Community Clinician to join our Hospice Outreach Service. As a key member of our multi-disciplinary team, you will play a vital role in delivering high-quality care to patients with life-limiting illnesses in the community.

You will have a strong background in palliative care, with experience in developing and implementing effective care plans that meet the individual needs of patients and families. You will work closely with other healthcare professionals to ensure seamless care coordination, and provide education and support to patients, families, and carers on palliative care options and symptom management.

**Essential Skills and Qualifications:**

  • Degree in Nursing, Paramedicine, or a related field
  • Postgraduate qualification in Palliative Care or equivalent
  • Proven experience in palliative care at Band 7 or equivalent
