Job Description
This is an exciting opportunity to join our team as a Care Coordinator. In this role, you will play a key part in ensuring the effective delivery of care services to approximately 60 service users in their homes. Your primary responsibility will be to oversee care plans, provide hands-on support when needed, and manage emergencies as they arise.
Responsibilities and Requirements
- Develop, implement, and review personalized care plans for service users to ensure consistency and quality of care.
- Coordinate care staff schedules to guarantee service coverage for all shifts and care requirements.
- Maintain effective communication with service users, their families, and healthcare professionals to ensure continuity and quality of care.
