The Hospice in the Weald's Hospice Outreach Service provides comprehensive palliative care and support to individuals with life-limiting illnesses in their own homes or care settings. As a Clinical Nurse Specialist/Clinical Paramedic Specialist, you will be part of a dedicated team working closely with other community teams and specialists to optimize patient experience and improve knowledge sharing.

Job Summary:

We are seeking an experienced professional with a proven track record of community or in-patient palliative care experience at Band 7 or equivalent level in related areas. You will have sound pharmacological knowledge of medications used within palliative care/EOL care and be able to work effectively in a fast-paced environment.


Key Responsibilities:
  • Develop and implement personalized care plans for patients and families.
  • Conduct regular assessments and reviews to ensure patient needs are met.

Requirements:
  • A degree in a relevant field such as nursing, paramedicine, or medicine.
  • Current registration with the relevant professional body.
  • Proven experience in palliative care or a related area.
