**Job Overview:**

We are seeking an experienced and dedicated Rapid Response Care Assistant to join our team at City and County Healthcare Group Ltd. As a key member of our multidisciplinary team, you will provide high-quality care and support to clients with complex needs, ensuring their dignity, comfort, and well-being.

**Key Responsibilities:**

  • Deliver personalized care and support to clients in their homes or community settings
  • Support clients with daily living tasks, such as bathing, dressing, and grooming
  • Collaborate with our team to develop and implement client-centered care plans
  • Communicate effectively with clients, families, and healthcare professionals

**Requirements:**

  • Previous experience in complex care, ideally with ventilation support, tracheostomy care, and peg feeding
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to travel to various locations within a 100-mile radius
